Patient InformationPrescription Refills Refills are expected to be provided at an office visit in all but certain extenuating circumstances. This is appropriate in order to allow proper review of a patient's medications to avoid errors and unsafe medication combinations. For this reason we also have decided unanimously to expect every patient to bring their ongoing prescription bottles with them to each appointment so they may be viewed and documented by our staff. In this way we can be certain of meds from other providers and remaining refills as well as a pill count on controlled medications when appropriate. The typical protocol from our office will be that each patient will be provided the appropriate number of refills to make it back to the office for follow up visits. In certain cases such as a new medication or dosage titration communication by phone may be appropriate for prescriptions. No refills will be provided during off-hours. It is our office policy that all controlled substances will be provided at an office visit unless directed otherwise by your provider. No refills of controlled substances will be provided after hours or on the weekend. Typically if a patient's symptoms are so severe as to require a prescription for a controlled substance, that patient needs to be seen. These policies are intended to allow us to provide excellent medical care and focus on managing medications appropriately for our patients.. Appointments Fees and Payments We participate in many company-sponsored insurance plans. Please check your provider book to determine if we are participating providers in your health insurance program. We are a participating provider in Medicare as well. Our billing staff will make every attempt to assist you with your account. However, it is very important for you to understand your insurance plan coverage and limitations. This will help prevent misunderstandings later. Patients without insurance are expected to pay at the time of service. Our physicians do not discuss financial issues with you or make payment arrangements. If you require special billing arrangements, those must be made with our billing department prior to the visit. We accept VISA, Mastercard, and most debit cards as well as check or cash. There is a fee for returned checks. Credit card payments may be paid by mail or phone. Records Transfer, Copy, and Release |
