What is a patient-centered medical home? It’s a model of care where you will have greater access to your personal doctor who coordinates your care through all stages of life, no matter your age or specific needs. In a patient-centered medical home, YOU are the center of the healthcare team. Your team is headed by your primary care doctor and includes all the healthcare specialists who help coordinate your care, including a nurse care manager to help you reach your goals.

All team members communicate through a shared electronic medical record system called MyChart, which you can also access to review your health history, check test results, and more. Since we firmly believe that good patient relationships are based on knowledge of methods and firm understanding of policies, we have also developed a page to share some of our basic office policies with you.
